Overview

This workshop will provide pharmacists with the knowledge and tools to be able to
  • systematically assess patient’s drug therapy,
  • define and implement patient specific care plans
  • document the provision of patient care.
The blueprint for pharmacy calls for pharmacists to manage patient’s drug therapy in collaboration with patients and their health care providers. In order to effectively carry out this responsibility, pharmacists must consistently and effectively assess patients’ drug therapy, determine patients’ drug-related needs, identify any real and potential drug therapy problems, recommend initiation and/or modification of patient’s drug therapy, document this care and follow-up to ensure that positive health outcomes are achieved for their patients.
Upon completion of this workshop, pharmacists will be able to:
  1. Define Pharmaceutical Care.
  2. Describe terminology used in Pharmaceutical Care practice and how this is consistent with health care practice.
  3. Define the components of the Patient Care Process.
  4. List the seven types of drug therapy problems.
  5. Describe the components of a care plan.
  6. Define how Pharmaceutical Care relates to Medication Therapy Management.
  7. Discuss various strategies which can enable effective implementation of care plans.
  8. Describe important considerations in setting up a Pharmaceutical Care Practice.
  9. Describe the impact of Pharmaceutical Care practice on patient outcomes.
  10. Discuss which important components related to patient care should be documented.

The workshop will incorporate small group learning case discussions and presentations.
Participants are encouraged to share their own patient care experiences from their practice during this workshop
